In my case i had 2 toddlers under the age of 3 and i absolutely needed that shit! Aside from my paranoia about my ability to keep them safe I walked alot when they were toddlers and babies & we visited theme parks and one of the biggest (if not the biggest) swap meet/auctions in the country almost every weekend. Thousands of people around all the time. Better safe than sorry! I also dont judge someone who doesn't use them.

I never used them and I don't judge anyone who needs or uses them. My kids are 5 years apart so it was never needed.

They come in handy for parents with special needs children. If a child has sensory disorder for example then they may not want an adult to physically touch them. Using a leash will give them a sense of freedom, not upset them and the adult still has control of the situation so they won't run off.
